‘Health Passport’ is a unique health scheme of Gujarat government, will tell the complete medical history of children.

Prime Minister Narendra Modi often emphasizes that the strong foundation of a ‘Healthy India’ can be laid only on ‘Healthy Childhood’. Putting this vision of PM Modi on the ground and towards realizing the dream of ‘Developed India’, the state government under the leadership of Chief Minister Bhupendra Patel has come up with the concept of ‘Health Passport’ to make the healthcare facilities for children and adolescents more smart, sustainable and family and child friendly.

School Health – Now under Rashtriya Bal Swasthya Karyakram (SH-RBSK) Gujarat Every child from birth to 18 years of age will have his own health passport. This will not be just an ordinary health card, but a solid document of the child’s entire health journey from birth to adolescence. It is noteworthy that this statewide program has been started on 27 June 2026 by Union Home and Cooperation Minister Shri Amit Shah in the presence of Chief Minister Bhupendra Patel.

Will prepare 992 mobile teams

Under SH-RBSK in Gujarat, health checkup of about 1.89 crore children is done every year by 992 mobile health teams. At present, the records of these checkups are available on the digital portal, but parents or children do not have any handy document which they can easily use for regular routine checkups, treatment or follow-up. The proposed ‘Health Passport’ will overcome this shortcoming and present the medical records of children in a systematic and easy format.

Passport will be available at the screening venue

Keeping in mind the convenience of the parents, this process has been kept very simple. Parents will not be required to submit any additional documents to get their children’s ‘Health Passport’ made. Health screening of children will be done by mobile health teams of SH-RBSK in Anganwadi, schools, madrassas, gurukuls and special schools. After screening and data entry on the digital platform, this health passport will be given to the children on the spot. Not only this, the system of its renewal is also clear. Health passports of children up to 5 years of age or out-of-school children will be renewed every year by the Medical Officer (MO) of the Primary Health Center (PHC). At the same time, this responsibility will be performed by the school principal for the school students.

‘Integrated record’ of health history

This health passport will be a master document of the personal medical history of every child. In this, along with the basic details of the child, complete records of age-wise health checkup from birth to 18 years will be recorded. Apart from this, the complete update of four major categories (4D) i.e. congenital defects, diseases, nutritional deficiency and developmental delay or disability focused under School Health-Rashtriya Bal Swasthya Karyakram (SH-RBSK) will be included in it.

Not only this, the child’s physical-mental growth and development, nutritional level, referral services, tips related to nutrition and health, lifestyle advice and emergency helpline number will also be a part of this passport. In simple words, families will get every small and big information related to the child’s health at one place.

If the physical health passport is lost

If for any reason the physical health passport is lost or damaged, a new health passport can be obtained by the mobile health team. The purpose of this health passport for children being brought by the Gujarat government is not just to prepare medical files, but to make parents an active partner in the health management of children. This will create a strong bond between children, parents and the healthcare system.
